Diabetes Education Program
Philosophy and Description of Service
The Diabetes Education Program, called Diabetes 123, is a three-week group-based program of two hours per week, run on the first three Tuesdays each month. It is run by the staff of Manningham Community Health Services Limited, including a Diabetes Educator, Dietitian, Podiatrist.
The Diabetes 123 course will cover:

Blood glucose monitoring and preventing complications |
A doctor’s referral is not required.
Other groups:
LIFE! A 5 session program for pre-diabetic people. |

Information sessions – 3 per year follow up from management group. Topics of interest related to diabetes. |

| Recharge – once per year reinforcing helpful habits needed to manager diabetes. | |
