Who else sees my information?

Your information can only be seen by the health care workers in this service who are involved in your care.
Otherwise, we only release information about you if you agree or if required by law, such as in a medical emergency.

How do you protect my information?

The law protects the privacy of your information. We treat your information in the strictest confidence and store it securely. We recognise that all client information held by us is sensitive.

Can I access my health record?

Yes, you have the right to request access to your information and to ask for it to be corrected if necessary.
Please contact our staff if you wish to access your health record.

What happens to my record if I no longer attend your clinic?

By law we are required to keep your health record in our archives for 7 years after you have completed your treatment at our service and then it is destroyed.  It is available for you view at any time during this period.  If a child under 18 has a health record we are required to keep it for 25 years before destroying it.

Please contact us if you have other questions or complaints about what happens to your information while you are our consumer.
