Philosophy and Description of service

Manningham Community Health Services Limited offers a comprehensive diabetes education and assessment service. The diabetes clinic and diabetes education service are  provided by our Diabetes Education Team. The team includes a Diabetes Educator, Dietitian and Podiatrist.

Manningham Community Health Service’s “Diabetes Service” aims to help you better manage your diabetes and is able to provide:

Advice about blood glucose testing
A review of your blood glucose monitoring technique
A range of blood glucose meters for sale
Advice regarding exercise
Nutritional assessment
Dietary advice
Assessment of nerves and the circulation in your feet
Foot health checks
Advice on footcare and footwear
Advice on managing stress
A structured five-week diabetes education plan
Life! Program for people with pre-diabetes

Eligibility Criteria

The service is for people with diabetes, their familes and carers. It is for people diagnosed with diabetes who want to update their knowledge or who are having difficulty managing their condition.

You do not need a doctor’s referral to use the service, but we do suggest you discuss your diabetes management with your General Practitioner
