Philosophy and Description of service
Manningham Community Health Services Limited offers a comprehensive diabetes education and assessment service. The diabetes clinic and diabetes education service are provided by our Diabetes Education Team. The team includes a Diabetes Educator, Dietitian and Podiatrist.
Manningham Community Health Service’s “Diabetes Service” aims to help you better manage your diabetes and is able to provide:

| Assessment of nerves and the circulation in your feet | |

| A structured five-week diabetes education plan | |
| Life! Program for people with pre-diabetes |
Eligibility Criteria
The service is for people with diabetes, their familes and carers. It is for people diagnosed with diabetes who want to update their knowledge or who are having difficulty managing their condition.
You do not need a doctor’s referral to use the service, but we do suggest you discuss your diabetes management with your General Practitioner
