In-home Services
This service is available to Home And Community Care clients, ie, frail older people, people with disabilities and their carers, who require home-based service.
Reasons to see a dietitian include unintentional weight loss or weight gain, decreased appetite, chewing/swallowing difficulties, diabetes and other nutrition related conditions, such as Parkinson's disease and Osteoporosis. A dietitian can help by:
Acting as a resource on food and nutrition issues. |

| Providing education on dietary management for clients and carers. |
