Eastern Hospital Admission Risk Program (HARP)
What is HARP?
The Eastern Hospital Admission Risk Program (HARP) provides integrated community-based care for people with defined chronic disease and complex needs. It focuses on integrated care in the community to reduce/eliminate avoidable hospital admissions.
The program’s key components are timely intervention and care planning involving the client, carer, clinicians, care coordinators, the patient’s GP and other medical specialists.
Who can benefit from HARP?
People who can use HARP and potentially reduce their avoidable hospital use include:
People with chronic heart disease |

What services does HARP offer?
All participants referred to HARP will have a case manager to co-ordinate integrated care, and appropriate services from allied health staff are funded in full, until local services accept the client.

HARP staff are co-located within community health services in the Eastern region, improving communication and referral for clients, who may be referred to their local CHS after the HARP intervention
What is the referral process for HARP?
Discuss your needs with a medical practitioner who can assist your referral to Eastern HARP Access.
Referrers can contact Eastern HARP Access by phoning 1300 661 141 or via e-referral (www.s2s.org.au) or fax 1300 661 145 using the Service Coordination Template Tool (SCoTT).
General Practitioners are able to use the GP statewide referral form (adapted from SCoTT). Eligibility, urgency and appropriate services can then be determined.
