Stride Forward Early Intervention Program

Child & Family Services

The Stride Forward Early Intervention Program provides individual programs for children, and family education and support.

Who is eligible for this service?

Families who:

Live within the Eastern Region, for example Manningham, Boroondara and Whitehorse.
Have a child aged between 0 and 6 years/school entry and who presents with a delay in two or more areas of development including:
-
Social/Emotional
  - Self-Help
  - Language/Communication
  - Gross Motor
  - Fine Motor
  - Sensory Integration
  - Cognition – Problem solving skills
  - Behaviour

The Stride Forward Early Intervention Team aims to provide high quality services based on the individual needs of the child/family, including:

Centre-based or Outreach programs consisting of “core services” such as Specialist Teaching, Speech Pathology, Physiotherapy, Psychology and Occupational Therapy
In partnership with the family, development of a “Family Service and Support Plan”
Individual Program for the child/family
Assessment and Reporting
Family Service Co-ordination (Case Management)
Inclusion support into Generic Services e.g. Preschool, Childcare, Family Day Care
School Transition Program
Waitlist Intake, Screening and Referral

Referral

Referrals to the Stride Forward Early Intervention Program can be made by:

Families/Guardians
Professionals including Paediatricians, Maternal Child Health Nurses, Early Childhood Professionals, Speech Pathologists, Psychologists, Physiotherapists, Specialist Children’s Services, or other agencies.
