Individual advice and support to manage your Diabetes

Our Diabetes Nurse Educator can provide tailored advice and support if you have Type 2 or pre-diabetes.

We will work with you in one-to-one appointments to develop a plan that’s right for you.


Advice, education and support allowing you to better:

  • Understand diabetes and make informed decisions and choices
  • Manage diabetes with advice and support with medications, meal planning, insulin and Blood Glucose Monitoring
  • Reduce complications that can be associated with diabetes

Live Well Stay Well with Type 2 Diabetes Group Program

This is a friendly, informal 2 hour group program for people with newly diagnosed type 2 Diabetes or anyone looking for a refresher about diabetes management.  This program is also suitable for people with Pre-diabetes.  If you would like further information about this group program click here.

Our fees are set on a sliding scale and are designed to support access for all income levels.

Difficulty or inability to pay will not be a barrier to receiving a service.
