Group session providing education for living well with diabetes
If you are looking for more information on how to manage your diabetes (or pre-diabetes), you can come along to a two hour group program.
Live Well, Stay Well – a Diabetes Nurse Educator will explain:
- What is diabetes?
- Myths and facts – why me?
- Monitoring of diabetes
- Answers to your questions.
The group session is held at 9:30am – 11:30am on the first Wednesday of each month (except January).
Run by our friendly Diabetes Educator.
Please note bookings are essential.
APPOINTMENTS AND REFERRALS
Anyone can refer to this program. Referrals can be made by phone, fax or electronic referral (email, S2S, Eref).
FEES AND FUNDING
Our fees are set on a sliding scale and are designed to support access for all income levels.
Difficulty or inability to pay will not be a barrier to receiving a service.
Services are offered via a range of funding streams which have varying requirements.
A GP referral may be required to access some funding streams (eg Medicare Benefits Schedule).
